If you so choose, I can provide you with a "superbill" which you can submit to your insurance for reimbursement up to 80% depending on your benefits.

1) Call the number listed on the back of your insurance card

2) Ask if your plan includes "out-of- network benefits"

3) Ask if you have a deductible for out-of network benefits for mental health care

4) Ask: "What is the maximum amount my plan will reimburse for mental health service code 90837 with a licensed clinical social worker?"
